A guide to our custom survey module to ask and collect the data important to your team

Our custom surveys module allows staff to ask athletes/users a wide range of survey and question types. From one off knowledge collection forms to regular bespoke touch points.

Creating a Survey

Survey Type:

To create a survey navigate to Apps -> Surveys then click NEW.

Now you can specify a few general details including survey name and type. The types of surveys include:

  • Daily Surveys (Require responses multiple times in a week e.g. rehab pain survey)

  • One Time Survey (Require one response e.g. Health Form)

  • Repeated surveys (Require specified response periods e.g. Quarterly performance reviews)

Survey Settings:

After the initial creation of the survey you will be sent to the settings screen to start setting up how the survey will work.

  1. Edit the survey name

  2. Survey Type is displayed

  3. Survey status

    1. Draft (When creating or editing the survey it needs to be in draft)

    2. Live (When you are happy with the survey and want to collect responses set it to live)

    3. Paused (When you want to stop collecting responses)

  4. Assign Participants

  5. Specify what roles can access the survey (You may want to send out a medical survey that only the medical staff can access)

  6. Specify Notifications (More on this later)

  7. Duplicate the survey (If you like the survey but want to send it to a different group you can duplicate it)

  8. Save your changes

Add/edit questions:

Navigate to the questions tab then click new or edit on a question to specify the question(s) to be asked.

  • Question type (What responses are you expecting from the question?)

    • Rating: sliding scale (1 to 10)

      • Specify range

      • Specify step distance (How precise should the responses be?)

      • Reverse the scale (Is a higher number worse?)

    • Yes/No

    • Text: free-text entry

    • Number

    • Multiple choice: single-select

      • Add Response options

    • Multiple choice: multiple-select

      • Add Response options

    • Instruction: a text block for instruction only, with no answer required

  • Question Text (How is the question to be asked in the survey)

  • Question Context (Additional information about the question)

  • Report Heading (Metric title in the results report)

When Updating a question you can change:

  • Change the question text

  • Change the Question Context

  • Change the question order

Response Periods

In the repeated survey type you can specify when you expect responses from the participants. As an example you can specify the last week in every month for monthly feedback surveys. Or you could specify each week for a weekly training goals survey.

Note: The system expects one response per period.

To specify a response period:

  • Navigate to the scheduling tab

  • Click new or edit

  1. Give the response period a name

  2. Give the period a description

  3. Specify the start and end dates you expect participants to respond in

Survey Notifications:

Once the survey is created you can specify when alerts are sent to the participants mobile to complete the survey.

  1. Navigate to the settings tab of the survey

  2. Under the NOTIFICATIONS section you can specify the notification schedule including

    1. Group (Default will go to all users in the survey)

    2. Day

    3. Time

Note: Reminder will only be sent inside a period that a user can submit in and only if they have not done the survey yet for that period.

Results

Head to the results tab of the survey.

This will then show you a list of the submission periods.

Click on a period to see the responses for that period.

To see results and how they change over time.
Navigate to the participants tab then click on the user you wish to study.
From there you can select the question to track that users responses over time.
